Unfortunately, there are many more people who need dogs than we can provide.

If you have not yet seen the limitations of who we work with and where we work, please read the following.

Honestly, one of the ways we use to find out who really wants a Service
Dog is to only help those who try to help themselves by completing these forms.

This diagram details the Assistance Dog Placement Process.

We understand this is not as easy task for someone who daily deals with PTSD, TBI,
or any number of other challenges.

Just do your best!

We recommend you open these documents in PDF (right click on the document and choose
‘open as PDF’. It is sometimes easier to fill these out online. You can save your work
on your own computer.

Upon completion of all required documents, mail or email them to us.
